An Innovative use of data to improve the care of COPD patients in Midlothian Community Respiratory Team (ID 505)

Abstract
A key challenge for the future is how to best utilise routinely collected population data to improve patient care. Achieving this through innovative data driven solutions is likely to be clinically and cost-effective, improve quality of life, reduce early deterioration, and treat patients in the right place and at the right time. We used a collaborative, creative approach working with clinicians, data and digital analysts, service and performance managers to create a data dashboard for Midlothian Community Respiratory Team (MCRT).Aims and objectives
To use routine data to provide a deeper understanding of teams’ impact,
-Explore the COPD patient data at population level and MCRT cohort level
-Illustrate the financial benefits of MCRT
-Visualise the teams impact using data
-Provide a platform of information on which to grow the service and facilitate continuous improvement projects.
Methodology
-Collaborative working between data and digital analysts and clinicians to create an innovative tableau dashboard product for CRT that has the potential to be scalable
-Local testing in MCRT (perform iterative improvements)
-Clinician training in dashboard use
-Integration of dashboard into business as usual (service level data collection)
-Evaluation of impact of dashboard data on clinical care (QI projects = extended working hours project, case finding project - current)
The benefits are:
-Time efficient - easy access to accurate CRT service data for reporting
-Use of dashboard to illustrate gaps in service provision
-Identify areas for quality improvement
-Improve understanding of Midlothian COPD patient cohort
-Visualisation of financial and clinical impact of CRT input
Conclusion
Using a collaborative approach in this project was key. This poster displays 2 of 8 pages of data insights. Transferring population level data learning to clinical practice is complex, required leadership and a clinical champion along with collaboration from local Midlothian data experts.
